  • Consumer Authorization Release
  • Voluntary Self Identification Form

HR Insights will assist you in building the Employment Application, Consumer Authorization Release Form and the Voluntary Self Identification Form. The Benefits Summary and Company Literature should be developed by your Company's internal management team.
