"What to include?"

Are there any state or federally mandated forms/documents that a new employee needs to complete on their first day?

The EMPLOYEE needs to complete Section 1 of the I9 form (Employment Eligibility Verification Form - http://www.uscis.gov/files/form/i-9.pdf) The EMPLOYER need to complete Section 2 within 3 business days from hire date. The reset depends upon what you need to get them on payroll, benefits, etc.
